NDIS
Vibrant Hummingbird is currently undergoing the process to become NDIS registered however customers who have submitted requests to NDIS have had great success.
The guidelines for purchasing sensory equipment using NDIS has changed.
Please follow the steps below if you wish to use NDIS funding to purchase a Vibrant Hummingbird pendant.
Step 1:
Request an invoice from us by emailing us admin@vibranthummingbird.com
You can also ask your plan manager to request this from us.
Details we will need for an invoice:
- Name of NDIS participant.
- Shipping address.
- How many pendants.
- What colour/colours.
- Plan managers will also supply us with your NDIS number to be listed on the invoice.
We are committed to protecting your privacy and securely store collected information for an invoice for 30 days from invoice date/payment due date. We may use it for fraud prevention, research, and analysis. We may also disclose it to relevant authorities as required by law.
Step 2:
When making a request to NDIS for sensory equipment, you must also provide evidence from an allied health professional to support your request. This could be from an Occupational Therapist, Physiotherapists, Speech and Language Therapists etc.
This evidence must clearly explain:
Adult - how the requested equipment will help you with personal care, safety or to do household tasks.
Child - will assist in achieving better long-term outcomes.
Step 3:
Ensure you forward the invoice and evidence to your plan manager if they have not gathered this for you.
